Poka-Yoke, from the Toyota Production System, makes errors impossible to commit or instantly detectable and stoppable when they occur. Instead of telling operators to be more careful, it changes physical conditions so mistakes become difficult or impossible. It is a high-priority item in automotive process audits and FMEA, and one of the most effective ways to eliminate human error. Customer audits and IATF 16949 require error-proofing on critical processes and verification that the devices still work.
The tool organizes the checklist by error-proofing level: prevention (errors physically cannot happen, e.g. foolproof interfaces and asymmetric locating), detection (immediate alarm and stop after an error, e.g. limit switches, photoelectric sensors and weight checks) and warning (alert only, e.g. buzzers and indicator lights). It also classifies items by detection target such as missing parts, reversed parts, wrong parts, missed operations and count errors, with typical devices like locating pins, sensors, counting scales and vision systems.
List critical stations and historical problem points, then select an error-proofing type and device for each failure mode and assess whether current protection exists, is effective and has check records. Pay special attention to bypassed or overridden devices, sensor verification and line-stop mechanisms when protection fails. Have process, quality and production teams confirm the checklist together so measures are real rather than paperwork, and review and score each item during audits.
Identify high-risk stations using FMEA risk ranking, record failure modes and existing protection station by station, evaluate effectiveness against the error-proofing level, then generate an improvement list and track implementation. Link the checklist with FMEA: FMEA identifies high-risk failure modes and the Poka-Yoke checklist turns them into concrete improvements. After implementation, verify results with defect data and replicate successful cases to similar stations, and use the checklist in layered process audits (LPA).
